Prior to participating on a team from Carroll Community College, athletes must provide the Athletic Department with current address, emergency contact, insurance, medical alert and health history information.

    • PLEASE READ BEFORE FILLING OUT: Any returners do NOT have to fill out the NJCAA affidavit
  • After you have completed the packet, please bring your documents to A118 (Student Engagement)

Guest Athletes

All students who attend ID camps or training sessions who are not on the team must complete a waiver and media release form. If a student is under 18, their parent must also sign the form.
